Lunch menus must be completed and returned with payment in order to be processed.

View and print the menu, check items you would like to order each day (mark with a number if you would like more than one). Be sure to put student’s name at the top.

Calculate the total for each day, and then calculate total for the two-week period. All lunch prices include a nominal fee which supplies paper products as needed and delivery.

Drinks= bottled water or choice of available bottled/canned drink.

Cash, check, Visa and Mastercard accepted. Credit cards will carry a 10% processing fee. Please make checks payable to ‘SRCS.’  Order form must include payment. Orders not received by due date cannot be guaranteed.

Due to complexity, we are not able to complete special requests other than those made available on this form.
